The Pottstown Area Health &Wellness Foundation’s cornerstone is its grants program. During fiscal year ’08, the Foundation plans to award approximately $3.1 million to schools and non-profit organizations in the tri-county area.

The following list includes grant recipients from the Fall 2007 grant round and describes how they are using the funds to benefit Pottstown area residents.

The Foundation distributes grants according to its four long-term goals:

  • Goal 1: Reduce Behavioral Risks
  • Goal 2: Improve Access to Medical Services
  • Goal 3: Enhance Formal and Informal Supports
  • Goal 4: Improve Physical and Social Environment

In support of the “Reduce Behavioral Risks” goal, the Foundation is donating $519,067 to the following 14 community organizations and programs:

Baptist Children’s Services – Support “BCS YES – Youth Empowered to Succeed”, a program for at-risk teen girls focused on promoting wellness.

Boyertown Area School District – Fund T.A.P.E. (Technology-based Assessment in Physical Education) for BEARS Initiative, physical fitness programs for teens, teachers and community members.

Child, Home & Community – Initiate adolescent, prenatal, parenting and support programs.

Fellowship Farm – Continue healthy options and life style programs in partnership with YWCA Tri-County Area for individuals, families and communities.

Holcomb Behavioral Health Systems – Support life skill program for vulnerable adults and their children.

Phoenixville Area YMCA – Provide movement and exercise classes for challenged children.

Phoenixville Area YMCA – Support Youth Training program to encourage physical activity, nutritional education and peer support.

Pottsgrove School District – Fund S.N.A.P. (Student Needs Assistance Program) Team which teaches healthy lifestyle choices.

Pottstown Area Seniors Center – Enhance Prime Time Health program

Pottstown Family Center – Continue Color Me Healthy, an early intervention nutrition and physical activity program.

Pottstown School District – Implement C.A.T.C.H. (Coordinated Approach to Child Health) Club.

Pottstown School District – Continue Safe Schools/Healthy Students program which provides information and programs on nutrition, fitness, drugs and alcohol, mental health and safety.

The Triskeles Foundation – Initiate Food for Thought program with Spring City community.

YMCA of Pottstown – Introduce Childcare Center Healthy Snack program.

In support of the “Improve Access to Medical Services” goal, the Foundation is donating $913,486 to the following four community organizations and programs:

Creative Health Services – Provide operational support for co-location of primary and behavioral healthcare.

Montgomery County Community College – Fund Dental Sealant Day to provide dental services to children in the Pottstown area.

Montgomery County Department of Health – Support breast cancer education and screening project for uninsured and underinsured women at Pottstown Memorial Medial Center.

Visiting Nurse Association – Fund Montgomery County Personal Navigator Program with expanded legal support service.

In support of the “Enhance Formal and Informal Supports” goal, the Foundation is donating $283,070 to the following six community organizations and programs:

Boyertown Area Creative Housing – Support Transitional Housing/Client Support program for families in the Boyertown area.

Mental Health Association of Southeastern PA – Fund Chester County Compeer, Inc.-  Fund program which matches rained volunteers with adult consumers of mental health services in the Pottstown area.

Montgomery County Community College – Improve service learning to build the capacity of community-based non-profit organizations.

Montgomery County Foundation, Inc. – Continue web-based resource directory which offers database of health and human service agencies and programs to Montgomery County residents.

The Pottstown Cluster of Religious Communities – Continue to fund Capacity Building Initiative.

Volunteer Home Care, Diakon Lutheran Social Ministries – Support volunteer home care team outreach.

In support of the “Improve Physical and Social Environment” goal, the Foundation is donating $477,500 to the following five community organizations and programs:

Blessed Teresa of Calcutta Parish – Provide matching grant for gymnasium building project.

Camphill Village Kimberton Hills – Support Aging in Community program which benefits vulnerable seniors with developmental disabilities.

Pottstown Area Police Athletic League – Support organization’s capacity building efforts.

Pottstown Parks & Recreation – Provide open space funding for parks, trails and grounds improvements, and playground improvements to seven small parks.

Pottstown School District – Fund planning process for school recreation facilities project master site plan.
