The Community Health Assessment was completed in June 2004 and approved by the Orphan’s Court of Montgomery County on Aug. 27, 2004. In the Jan. 15, 2005 edition of The Mercury newspaper, the Foundation shared results of the health assessment and its subsequent community plan as a service to the tri-county region. As the Foundation approaches its five-year anniversary we are planning our second needs assessment.  This information will be instrumental as we reflect on our past and plan our future in promoting health and wellness within the tri-county area.
