The Pottstown Area Health & Wellness Foundation is a community organization created as the result of the sale of the Pottstown Memorial Medical Center to Community Health Systems. With net assets of $79 million as of April 24, 2012, the Foundation serves the residents of Pottstown borough and those within a 10-mile radius of the borough.
The Foundation’s purpose is to provide opportunities for our community to enthusiastically embrace a healthier lifestyle. We’re here to offer the tools and motivation for people to improve their health and well-being every day.
The Foundation is a non-profit organization dedicated to assuring increased access to health and wellness education and services. We provide these services directly through our own programs and through other non-profit health and wellness providers.
The Foundation provides grants to non-profit health and wellness providers, who use the money to develop and enhance programs that support our purpose. We take an outcomes-based approach to grant-making. Outcomes are things that benefit people, such as new knowledge, increased skills, positive change in attitude or values, positive change in behavior or an improved condition. We award grants to area organizations that best use their resources, services and products to help people improve their health and well-being.
In short, we’re here to help area residents improve their health and well-being every day. By doing so, we are creating a more dynamic and more vibrant community in which to live.
